Herniated Disc — a diagnosis that millions of people worldwide receive every year. Traditionally, patients are offered two paths: conservative treatment (medications, physical therapy, injections) or surgery. But does a third path exist — true disc regeneration?
The Scale of the Problem
According to the Kassenärztliche Bundesvereinigung (KBV), musculoskeletal diseases are the #1 reason for doctor visits in Germany — more than 36.7 million patients annually. Herniated disc is one of the most common causes of chronic back pain.
In Germany, approximately 150,000 intervertebral disc surgeries are performed annually; in the USA, more than 500,000. However, research shows that up to 40% of patients do not experience expected relief after surgery (Failed Back Surgery Syndrome).
Why Doesn't Conservative Treatment Solve the Problem?
Standard conservative treatment includes:
- NSAIDs — mask pain but do not affect the herniation
- Physical therapy — strengthens muscles but does not regenerate the disc
- Epidural injections — temporary relief (2–4 weeks)
- Manual therapy — relieves muscle spasm but the herniation remains
All these methods work with symptoms, not with the cause. The damaged disc continues to degenerate.
Regenerative Methods: What Does Science Offer?
PRP Therapy
Platelet-rich plasma (PRP) — the first step in regenerative medicine. The method uses growth factors from the patient's own blood. The problem: the concentration of growth factors in standard PRP is insufficient for fibrous ring regeneration.
CGF + Lipogems® (MIBRAR® Method)
The MIBRAR® method uses two types of autologous concentrates:
- CGF — concentrated growth factors from blood (5–10 times more effective than PRP)
- Lipogems® — mesenchymal stem cells from adipose tissue
A combination of these concentrates, delivered directly to the damaged disc through a 0.3–1.5 mm micropuncture under 3D navigation with Cyber Navi Hand™, initiates true tissue regeneration of the disc.
Results of Herniated Disc Treatment Using MIBRAR® Method

How Does Disc Regeneration Occur?
The intervertebral disc is an avascular structure (without blood supply). This is the main reason why it cannot heal on its own. MIBRAR® solves this problem biologically:
- Intradiscal delivery — concentrates are injected directly into the nucleus pulposus through a 0.3 mm micropuncture under 3D navigation with Cyber Navi Hand™
- Stem cells → chondrocytes — mesenchymal stem cells from Lipogems® differentiate into disc cells and synthesize type II collagen and proteoglycans
- Restoration of hydration — proteoglycans retain water, the disc "swells," restoring height and shock-absorbing function
- Fibrous ring healing — CGF growth factors stimulate healing of the tear through which extrusion occurred
Follow-up MRI in 8–16 weeks shows reduction of herniation and restoration of disc signal (hydration).
Who is Suitable for Non-Surgical Treatment?
The MIBRAR® method is effective for:
- Disc protrusions and extrusions of any location (cervical, thoracic, lumbar)
- Herniated discs up to 15 mm (including sequestered — in select cases)
- Multiple herniated discs (treatment of multiple discs in one procedure)
- Recurrences after surgery (Failed Back Surgery Syndrome (FBSS))
